Under medical and prescription drug plans, certain medications may need approval from your health insurance carrier, before they’re covered. If your pharmacy states that you need a prior authorization to get your prescription filled, please fill out the form below. Your request will be processed within 2-3 business days. As your mental health provider, we will advocate and do our best to get your medication covered but ultimately it is your insurance policies decision to approve or deny coverage.
