This can be done at home at your convenience so you do not need to stress/rush filling paperwork out in the office. The check-in forms are very valuable to the practitioner's treatment plan so we thank you for your time and patience.

Please Read before scheduling:
1. Only existing patients are able to schedule online. If you’re a new patient, please view the New Clients tab above.
2. The office will call you as soon as possible if there is a conflict with the appointment you scheduled online. Please pay close attention to your text appointment reminders.
3. If you are unable to keep an appointment, please notify our office immediately. This time is reserved exclusively for you. If an appointment is missed or canceled without 24 business hours' notice, you will be charged a $100 no-show fee for medication management and/or therapy appointments.
4. Do not try to schedule the day you need a refill on your medication. This may result in missed doses due to scheduling or pharmacy issues.
